RSHOL #9505 Root Cause Analysis-Accidents & Injuries   

Course Description:

This course will assist participants in delving further into the symptoms of an accident, near miss, or incident to determine the true cause (Root Cause) of the problem. This course will explain how to learn the most from accidents and near misses; and, how participants can contribute to an effective business improvement process. Two primary topics will be addressed the 5 Why’s and Root Cause Analysis.

Prerequisites: OSHA #7505 Introduction to Incident (Accident) Investigation

NOTE: This is an asynchronous, online class within Vol State’s Learning Management System. There are no on-campus meetings.

Course Topics/Outline:

  • Define Root Cause Analysis
  • Explain why Root Cause Analysis is important to OSHA
  • Define the Problem
  • Identify the causes of the problem?
  • Explain what the different types of methods are available to conduct RCA
  • Explain the 5 Whys
  • Design a solution to a problem
  • Explain how to implement the solution
  • Identify techniques for evaluating the results
  • Explain why to monitor the solution over period of time.
  • Solve a Capstone case using the Fishbone Diagram
